Class II division 1 adolescent treated Non-Extraction with aligners and Class II elastics and Orthopulse

My goal was to reduce her excessive overjet and correct the Class II bite while respecting her family’s preference to avoid braces. She had already spent almost two years in aligner treatment with limited improvement, so I focused on revising the mechanics, attachment design, and anchorage strategy. I chose to continue with Invisalign using staged upper molar distalization and Class II elastics, with her remaining growth potential incorporated into the plan. The key was coordinating the tooth movements with adequate support to limit unwanted forward movement of the upper incisors. Patient education became an essential part of treatment. Early inconsistent elastic wear was accompanied by further flaring of the front teeth and increased overjet. I explained how moving the back teeth backward creates reciprocal forces elsewhere in the arch, and why her elastics were an important part of controlling those forces. Once she understood their purpose and wore them consistently, the overjet began to improve. Because she wanted to shorten the additional treatment after her previous experience, we included OrthoPulse as an adjunct and used a closely monitored aligner change schedule. The foundation of the correction remained the treatment plan, attachment design, staged movement, and consistent elastic wear. After 14 months, we achieved Class I molars, 2 mm overjet and overbite, and an improved profile without extractions or fixed appliances. This case highlights why successful aligner treatment depends on both the prescribed mechanics and the patient understanding how to use them.
